The forms linked below contain the permission slip that is required, by East Granby Public Schools, for your child to attend this program. Please choose the appropriate form for your child, print and complete it, then sign the permission slip. Scan the completed form or take a clear, well-lit photo, and email it to eastgranbyrecreation@gmail.com, no later than one week before the program begins. Failure to submit a permission slip on time may forfeit your child's spot to a student on the waiting list.
